Sort of like the first option, except the Doctor does not die and
reincarnate.  Time Lords "regenerate."  From memory, they can regenerate
due to a tramatic event.  From what I can tell, the event does not always
have to be a physical trauma.  It seems like a couple of the recent ones have
regenerated after some form of emotional trauma.
