About the forms folder. 

This is a collection of user generated forms for UCSD administrative 
activities like evaluations and such. These forms are here for the 
convenience of the UCSD Community and are probably current as of 
the date of last modification. Forms are in many formats like Excel, 
Filemaker Pro and Microsoft Word.

There is no support or answers to questions about about these forms, 
they are freely given to support the UCSD community. This is based 
on a series of mail exchanges in the managers@ucsd.edu listserve 
that expressed the need, and those that were willing to share their work.
